Primary Care access
Clients have access to a Federally Qualified Community Health Center on site. The clinic receives patients by appointment and walk-in. Services include screenings, primary care services including care for diabetes, high blood pressure, and other chronic diseases. This clinic also provides routine wellness visits and wellness counseling. Referrals and pharmacy coordination are available to clients who require these services.
Behavioral Health Care
Clients have access to a dedicated, Health Care District licensed behavioral health specialist on site. Clients can request a meeting with the specialist to address issues and follow up with further care if needed. A psychiatric nurse, also provided by the Health Care District, is also available on specific days.
Substance Abuse Care
Clients have access to an onsite outreach coordinator if they need help with substance abuse. Clients are referred to the MAT (Medically Assisted Treatment) clinic if they are struggling with drugs and alcohol, who want help. Transportation is provided if needed. The goal is to eliminate any barriers for someone if they want assistance. This service is provided by Health Care District of Palm Beach County.
District Cares Voucher Program
Clients are able to sign up for District Cares Specialty Benefits. The specialty benefits are available to uninsured residents of Palm Beach County who do not qualify for any other public assistance health coverage program and who meet income and residency requirements. They will meet with a financial counselor who will assist them in applying for the benefits.

St. Ann Place Outreach Center is a homeless resource center located in West Palm Beach, Florida. St. Ann Place provides homeless services including assisting clients with obtaining identification, showers, laundry services, meals, and mail services. St. Ann Place Outreach Center is organized exclusively for charitable, purposes under section 501(c)(3) of the Internal Revenue Code.
